Every service starts with an assessment

Each clinical service begins with a nursing assessment and a written care plan. We agree the goals with you, set a visit schedule that suits your household, and document each visit so your GP and funding body always have a clear picture.

If something changes, we act. Our nurses escalate concerns promptly, contact your treating team and keep your family informed, so small problems do not become hospital admissions.

When care turns to comfort

Palliative support at home asks for a particular kind of nursing: unhurried, calm and honest with families about what is happening. We work alongside your palliative care team, and we do not rush the visit.
